All student records are kept confidential and will only be released upon receiving a written request from the enrolling parent(s) or legal guardian of the student. If you desire to review your child’s records, please call our office and an appointment will be made for you to review the record.

 

 

 

Upon receipt of tuition and Request for Transfer of Records form for the student, 3J4 Academy will request that records be sent from the student’s previous school. Records received from previous schools generally include attendance, standardized test scores and transcript. For students who have had special education, an IEP is usually included, as well. Schools will not include in transferred records information pertaining to behavioral problems. If a student has experienced suspension it is the responsibility of the parent to include such information in the enrollment process.

 

 

 

Reporting of Records

Each family must submit by no later than January 15 and July 15 a completed attendance and grade report form.  Reporting forms can be downloaded and printed from our website:  www.3j4academy.com. Attendance will be documented on the form and an average grade will be reported for each subject.

 

To average grades for a subject, add all test grades for the subject and then divide the sum by the number of tests.  Example, a certain student’s science test grades are: 85, 92, 90, and 95.  Add them together, and the sum will be 362.  Divide 362 by 4, the number of test grades.  362/4=90.5.
